I think it is Vitamin K 1. I don’t have the bottle anymore, but you can check it out on Amazon’s page. There are specifications regarding «Active Ingridients» and «Full Ingridients».

Do these come in an actual bottle or simply a sealed bag?

The ones that I received came in a plastic tub with a popup airtight lid; not very good if you have problems with your fingers and manual dexterity.  However I did buy these around a year ago and at that time the picture in the advertisement depicted this accurately.  I now see that the picture of the container has changed to one with a screw top.  I cannot say if this is a true representation; however, from my past experience I would say that it will be.  If your main concern is that it may come in a sealed bag I can definitely confirm that my purchase did not and came in a seal-able hard container as stated previously.
